NEET Admit card in Original.
NEET Result/Rank Letter.
Original mark sheets & Pass certificates of class 10th
Original mark sheets & Pass certificates of class 12th
MBBS Degree Certificate/Provisional Certificate.
MBBS mark sheet all year.
Internship complete Certificate
Permanent / provisional Registration Certificate issued by MCI / State Medical Council.
College Leaving Certificate in Original.
Conduct Certificate in Original.
Migration certificate in original.
Aadhaar Card of the Candidate.
PAN Card of Candidate.
10 Nos of colour Photograph.
2 sets Photocopy of all the above document
